Default

The video was extremely low resolution but it looked to me as if puffs of black smoke intermittantly came from the rear of the aircraft as if something had been taken into the engine.

Post Investigator's findings

According to the official report, there was:

Quote:
... clear and convincing evidence that distorted data introduced into the Mishap Aircraft's flight control computers caused an uncommanded, 30-degree nose high pitch up on takeoff resulting in a stall and subsequent crash.

Moisture in the port transducer units during an air data calibration caused an unneccessarily large 'bias' or correction to the air data system.

....

Another substantially contributing factor was the ineffective communication of critical information regarding a suggested technique of turning on pitot heat in order to remove moisture before performing an air data calibration.
