Default VH-QPJ A330 incorrectly loaded

Came across this incident on the ATSB Aviation website.

In July 2015 VH-QPJ was being loaded at Bangkok Airport for a flight to Sydney. The loading supervisor was at Bangkok and the load controller was in Warsaw Poland.
A pallet, weighing 2785 kg was loaded into position 23P. After reading the loading to the load controller the loading supervisor was directed by the load controller to offload the pallet. The pallet was not offloaded and remained in position 23P.
The load controller prepared the final load sheet, minus the pallet in 23P, and this was sent to the flight crew. This data was then used for reference take off speeds, fuel burn etc. The aircraft took off.
It was later noticed that the pallet was on the aircraft and the flight crew were notified.
