Default Pilot blamed for Adam Air crash

An inquiry into a major plane crash in Indonesia which killed more than 100 people has found that pilot error and a faulty navigation device were to blame.

The plane, a Boeing 737 operated by Adam Air, crashed into the sea on New Year's Day last year.
