Same thing happened to Air Canada today, C-FNNH Rear Door starboard side, aircraft was delayed for about an hour whilst a replacement slide was fitted.

Out of curiosity, how did this happen? I'm assuming someone tried to open the door from the inside? If I have read everything correctly when you push the 'red flap' which you need to do to open the door it is automatically disarmed.

I am more leaning towards the caterers...not following procedure considering they have both occurred on the rear doors...

Caterers don't usually load from the left side of the jet...in OGT's case.
