Airline safety standards & maintenance: getting better or worse?

It is difficult to address this issue on an industry-wide basis (with each and every airline's safety standards taken into consideration) but overall, are maintenance and safety standards improving or decreasing?

Competition is intense and cost-cutting measures are a reality, but is the state of the industry getting worse, as some people claim?
