Same thing happened to Air Canada today, C-FNNH Rear Door starboard side, aircraft was delayed for about an hour whilst a replacement slide was fitted.

Out of curiosity, how did this happen? I'm assuming someone tried to open the door from the inside? If I have read everything correctly when you push the 'red flap' which you need to do to open the door it is automatically disarmed.

The doors will automatically disarm if opened from the outside, unless there is a fault with the mechanism.
