Question All Nippon flipover

What switch could possibly cause this incident? It sounds most implausible to me:

ABC reported it was the rudder trim knob which is located on the same panel of the centre pedestal as the cockpit door controls.

You would imagine that if it was uncommanded rudder hardover ANA wouldn't come out so quickly and blame the copilot...

Doesn't sound like an impossible thing to happen if he was distracted talking to ATC or flying the plane (autopilot/FMC changes) etc. Still poor form.
