I've actually had frequent flyer points earning denied on a fully paid, fully flexible business class ticket.

The ticket was with a partner airline of the FF program, but the code they used for the fully flexible business class was the same code as the host airline used for one of its award ticket classes.

Took 4 phone calls and a few letters to sort it out, so this kind of stuff can be confusing even for those who work in the industry.

Conversely I have rewarded FF miles while using a redemption ticket

flight was booked by AA and redemption ticket was issued by AA, but flight was on CX, and CX gave me the miles nad it was in F class!!!
